Who This Page Is For

Policyholders sending paperwork on a claim that is already open. That is unusually direct for an insurer: no provider-only carve-out, no separate biller line. Anyone acting on your behalf can use the same number as long as your claim number is on the pages. If you have not reported the loss yet, this is not the right route.

Which route should I use?

  1. Check whether the claim exists yet. No claim number means no fax, report the loss first and get the number.
  2. Separate claim paperwork from policy paperwork. An insurance card request sent to the claims fax lands in the wrong queue.
  3. Put the claim number on every page, not just the cover sheet.

What to include

A claim packet is usually heavier than expected: a repair estimate, photographs, receipts, and sometimes a signed form. Send it as a single transmission with a claim cover page carrying your name, claim number and callback number. Photo-heavy claim evidence is exactly the case where sending a large file without shrinking it into illegibility matters.

FAQs

Why does the paperwork say AFICS and not American Family?

AFICS is American Family Insurance Claims Services, Inc., the claims organisation inside the American Family group. The document email address and the Madison mailing address both carry that name, so correspondence coming back to you may be branded AFICS. It is the same company handling your claim.

Is this the same fax number for a Costco or Connect policy?

Yes. American Family lists the identical claims document fax across its different distribution channels, alongside the same shared claims mailing address in Madison. The customer service phone number differs by channel, but the claims document intake does not.

What if I do not have my claim number yet?

Get it before you send anything. American Family's own instruction is to include the claim number on all correspondence, and a fax that arrives without one cannot be attached to your file. The 24 hour claims line, 1-800-692-6326, will give you the number.
