Sentry Insurance publishes one fax number, 800-999-4642, dedicated to the Sentry Insurance Group California Medical Provider Network. Two teams share the line, so the fax must name which one it is for: MPN medical access assistants, or MPN Contacts. This is a network access and administration fax, not a billing fax.

No. The Minnesota provider requirements exist because of Minnesota SB2193 and are entirely electronic: Jopari for medical and dental with payer ID J1417, Optum for pharmacy. No fax number appears on the Minnesota page at all.
