Chubb accepts workers compensation injury reports by fax at 1-800-664-1765, 24 hours a day, seven days a week. This is a first-report-of-injury line: it is used by the employer to notify Chubb that a workplace injury has occurred. It is not a general claims fax, and it is not the number for sending documents into a claim that is already open.

Chubb asks for the policy number, the date of the injury, the location, and a short description of what happened. If the claim is already open, the claim number must appear on every page you send, because that is what routes the fax to the right adjuster. If you are sending from a desk rather than a machine, faxing from your computer keeps the whole packet in a single transmission.
