Aetna splits its prior authorization fax numbers by drug type and plan rather than publishing one general number. Specialty drug requests fax to 888-267-3277, non-specialty drug requests to 877-269-9916, and Medicare Advantage requests to 844-268-7263. A clinical-only intake line, Aetna's FaxHub, takes certain requests at 833-596-0339. Which route should I use?

Check the member's ID card first, plan type (commercial, Medicare Advantage, or a state D-SNP) determines which fax number applies.
Split specialty and non-specialty drug requests, they go to different numbers even for the same member.
If you are the member yourself, none of these numbers apply to you, use the channel Aetna built for members instead.

Documents to include

Use the precertification request form (form code GR-69374-3) for drug and medical requests, it asks for the member ID, prescriber information, and clinical justification. Because these faxes carry protected health information, a HIPAA compliant fax setup matters here.
