MetLife uses separate fax destinations by claim type. Group life insurance claims fax to 570-558-8645. Disability and absence management claims fax to 800-230-9531. Employer-submitted annuity paperwork faxes to 877-549-5834, while annuity claims and beneficiary requests use 877-547-9669. For dental claims, the TakeAlong Dental FAQ lists fax 859-389-6505 for completed TakeAlong PPO claim forms. This is not a universal MetLife dental fax.

Which route should I use?

1. Identify the line of business first, life, disability, or annuity, each has its own dedicated number.
2. For annuities, check whether the paperwork is coming from an employer/plan sponsor or directly from a beneficiary, the two use different numbers.
3. For dental claims, identify the plan before selecting a destination; use 859-389-6505 only for the TakeAlong PPO route described above.

Documents to include

Include the policy or certificate number on every page, MetLife's claim forms are specific to each line of business and are available on the same File a Claim page. A beneficiary filing an annuity or life claim should also have a certified death certificate ready to submit alongside the claim form.
