What is Anthem's fax number for prior authorization?

Anthem does not publish one national prior authorization fax number, coverage is split into state-licensed plans, and each maintains its own routing. In California, Medi-Cal prior authorization requests fax to 800-754-4708, with a separate behavioral health line at 855-473-7902. Anthem's Cal MediConnect plan became a Medicare Medi-Cal plan on January 1, 2023. Anthem's California provider site still lists medical notification and prior authorization for the former Cal MediConnect plan at 888-235-8468, and the former plan's member appeals fax has been listed as 888-458-1406 (not confirmed on a current Anthem source), so confirm both before you fax. Anthem also increasingly pushes providers toward its Availity portal instead of fax. Elevance Health is Anthem's parent holding company, renamed from Anthem, Inc. in 2022, but the plans themselves still operate under the Anthem brand.

Which route should I use?

1. Identify the exact state and plan type on the member's ID card, Anthem's numbers are licensed and routed by state.
2. Check whether Anthem's Availity portal covers the request, Anthem is actively steering providers there instead of fax.
3. Don't assume a number confirmed for one state, like California, applies elsewhere.
