Which route should I use?

1. Check uhcprovider.com's portal first, UnitedHealthcare has explicitly moved most prior authorization there.
2. If your state and plan combination isn't one of the few still using fax, don't search further for a number, none is published.
3. For the states and plans that do still fax, use the exact number for that plan type, they aren't interchangeable across states.
