Is fax still accepted at Express Scripts?

Express Scripts, part of Evernorth/Cigna, confirms on its official prior-authorization-resources page: "If you're unable to use electronic prior authorization, there are other ways to submit your PA request... download a state specific fax form or fax your requests to the number shown on our general request form." One confirmed number, 1-877-328-9660, covers the Benefit Coverage Request Form used for ACA preventive-drug coverage reviews. Most other numbers appear only on state-specific PDF forms linked from the same page, not printed as general page text.

What replaced fax at Express Scripts?

Nothing has replaced fax outright, it remains an explicit fallback. Express Scripts' own page promotes ePA through eviCore, Surescripts, and CoverMyMeds as the primary channels, using the phrase "unable to use electronic prior authorization" as the condition for falling back to fax or phone.

Which route should I use?

1. Try ePA first through Surescripts or CoverMyMeds, this is Express Scripts' promoted default.
2. If ePA isn't available, download the state-specific fax form for the request type, the fax number is printed there.
3. Use 1-877-328-9660 only for the Benefit Coverage Request Form (ACA preventive-drug reviews), not as a general-purpose number.
