Evolent prior authorization routes depend on the health plan, state and service. RadMD is recommended; check the applicable plan instructions before faxing.
Who This Page Is For
Providers whose health plan routes a prior authorization request to Evolent. Verify the member’s plan and service before choosing the submission route.
There is no single answer for every plan or service. RadMD’s contact guidance recommends the portal but also permits requests through the health plan’s designated prior-authorization phone number.
Do not treat the absence of a general corporate fax as proof that all fax intake has ended, or use a support phone number as a prior-authorization fax.
Several other pharmacy prior authorization intermediaries follow a similar pattern. Our directory also covers Cohere Health's fax number, is fax still accepted at Express Scripts, the MedImpact fax page, and Accredo's prior authorization fax, each with its own fax status and routing notes.
Fax.Plus stores RadMD.com supplementary documentation faxes with 256-bit AES encryption at rest, unique key per user.
Check the member’s health plan, state and requested service. RadMD is recommended, but designated plan-specific phone routes also exist. Any fax route must be confirmed against the applicable current form or case instructions; neither a universal fax ban nor a universal fax number is established. See RadMD’s guidance.
RadMD is a provider portal for initiating and tracking supported prior authorization requests. It is not the only route for every plan: designated prior-authorization phone routes vary by health plan. Follow the instructions for the member’s plan and service.
