Medical bills and supporting records on auto and workers' compensation claims go to (844) 218-1140. Claims against State Farm Health, the line that includes Medicare Supplement business, go to (844) 649-6986. Both numbers live on State Farm's business-to-business portal, which is the clue to who they are for: these are provider and biller routes, not policyholder routes. State Farm does not publish a general claims fax number for customers on statefarm.com.

Which route should I use?

1. Check which State Farm entity is actually paying. A bill from a car accident or a work injury is auto or workers' compensation, and goes to (844) 218-1140. A bill under a State Farm Health or Medicare Supplement policy goes to (844) 649-6986.
2. If you are not a provider or a biller, stop before you dial. Neither number is a customer service line.

What to include on the fax

Bills submitted on the auto and workers' compensation route follow standard medical e-billing practice: put the Property and Casualty claim number in Box 11b of the CMS 1500 form. State Farm also splits its claims mailing addresses by state, with Florida, North Carolina and West Virginia going to a different PO Box than everywhere else. Because these are medical records moving to a payer, the fax carries protected health information, worth deciding about deliberately. A HIPAA compliant fax setup matters here in a way it does not on a consumer page.
