- Farm, ranch, or other Agribusiness policy with a claim form filled in: 1-800-842-1482.
- Commercial policy reporting a new loss: 1-800-554-2899, with the loss-reporting form plus supporting documents in the same send.
- Workers' compensation specifically: the same commercial number handles the employer's loss report; medical bills are separate.
- Personal auto, home, or life: there is no fax. Use 1-800-421-3535 or the online account.

Nationwide Workers' Compensation accepts eBill submission in all states and on all claims, through your own clearinghouse or WorkCompEDI at (800) 297-6909, payor ID LV 164. No provider fax number is published, faxing a bill is not a supported route here. For the completed claim form itself, work from your computer where the form was completed.

LV 164. Nationwide states it covers Nationwide, Allied, Harleysville and Nationwide Agribusiness, so the same ID applies across those brands.
Where do I get the loss-reporting form to fax?
Nationwide's workers' compensation virtual toolkit holds the state-specific documents employers need, and your agent can supply the form as well. You can also report the same claim by phone at 1-800-421-3535 or by emailing the form to ENEWLOSS@nationwide.com.
