Does Allstate have a claims fax number?

Not a general one. Allstate's claims support pages list the mobile app, My Account, the claims line at 800-255-7828, and a mailing address at PO Box 660636, Dallas, TX 75266. No fax number appears on any of them. The one fax route Allstate publishes and stands behind is narrow and statutory: New Jersey personal injury protection medical billing, administered by Allstate's vendor Medlogix, with medical bills going to 609-631-0925. Everything else described online as an Allstate claims fax comes from somewhere other than Allstate.

Which route should I use?

1. Are you billing an Allstate New Jersey auto claim as a provider or health carrier? Match the number to the action, bills, precertification and appeals do not share one destination.
2. Is the claim outside New Jersey, or the document anything other than a medical bill or appeal? There is no fax route, upload through My Account or the app.
3. Did you find a fax number on an Allstate agent's page? That is the agent's own office line, not a claims destination.

The most expensive wrong turn here is trusting an off-domain number. Several widely circulated Allstate claim fax numbers trace back to law firm pages and old social posts. Allstate's current claims pages offer no fax at all, so an off-domain number cannot be checked against anything.

Documents and prerequisites
