Which route should I use?
- If you are an employer and an employee has just been injured at work, fax the report to 1-800-664-1765, or call 1-800-699-9916. Chubb asks that injuries be reported within 24 hours of notice.
- If a claim already exists and you need to send paperwork, call your adjuster and ask for the fax number assigned to that claim, or use Chubb's online claim document tool.
- If you are an injured employee, report the injury to your employer. Your employer files with Chubb.
What to include before you send
Chubb asks for the policy number, the date of the injury, the location, and a short description of what happened. If the claim is already open, the claim number must appear on every page you send, because that is what routes the fax to the right adjuster. If you are sending from a desk rather than a machine, faxing from your computer keeps the whole packet in a single transmission.
