Memorial Hermann operates one centralized Release of Information Department for all its Houston-area hospitals, confirmed on its own official page. That page, and the current live patient records-request form, do not publish an inbound fax number for submitting a records request, only phone and mail. Imaging records (films and CDs) have their own separate, confirmed fax.
