Oregon's Workers' Compensation Division accepts Request for Hearing appeals by fax. New injury claims are not filed with WCD directly.
Who This Page Is For
Workers appealing an Oregon workers' comp decision by filing a Request for Hearing. This is not the route for reporting a new workplace injury.
Oregon's Workers' Compensation Division does not take initial injury claims by fax, or by any other direct channel. Form 801, the new injury claim, goes to your employer, who forwards it to the insurer, confirmed on WCD's own worker filing page. What WCD does confirm by fax is the Request for Hearing (Form 2839), stating on the official form itself: "Fax to: WCD Hearings, 503-947-7511."
Fax number verified 2026-09-17.
Working a claim elsewhere? See sending a fax to New Jersey Temporary Disability or the Texas DWC fax page.
No, new injury claims (Form 801) go to your employer, who forwards them to the insurer. WCD itself doesn't take initial claims by fax or otherwise.
Filing a Request for Hearing (Form 2839) if you're appealing a workers' comp decision, not for reporting a new injury.
