Is fax still accepted at Evolent?

Evolent has moved prior authorization intake to its RadMD.com provider portal. Evolent's own operating platform documents instruct providers to "initiate an authorization request, Visit RadMD.com," and mention faxing or uploading supplementary clinical notes only after a request is already open, never as the way to start one. Evolent's corporate site lists no PA fax number at all and routes providers to the portal instead.

Which route should I use?

1. Go to RadMD.com to start any new prior authorization request, don't search for a fax number.
2. If a request is already open and a reviewer asks for supplementary documentation, RadMD.com's case page will show the specific fax number to use for that case.
3. Numbers found on third-party health-plan pages for Evolent are not confirmed on evolent.com, treat them cautiously.
