CVS Caremark's Prior Authorization Department accepts fax submissions for drug coverage reviews, step therapy exceptions, and non-formulary requests. The correct number depends on the member's plan type, confirmed directly from CVS Caremark's own prior authorization contact page:

Medicare Part D: Fax 1-855-633-7673, Phone 1-855-344-0930. For a Medicare Part D coverage determination, check the member's specific plan website for the required form.
Medicaid: Fax 1-866-255-7569, Phone 1-877-433-7643. A Minnesota-specific Medicaid PA form is published separately; other states may have their own.
Non-Medicare (commercial): Phone 1-800-294-5979. The fax number, 1-888-836-0730, is not listed on the main contact page, it appears on the linked Global Prior Authorization Form itself, easy to miss if you only check the contact page.

Which Number Should I Use?

Check the member's ID card or plan documents for Medicare Part D or Medicaid first, those two plan types have their own dedicated fax lines. If the plan is neither (a standard commercial or employer plan), use the Non-Medicare fax on the Global Prior Authorization Form. Sending a commercial request to the Medicare Part D fax (or vice versa) will delay the review, the departments don't cross-route requests.
