What is Aflac's fax number for claims?

Aflac publishes a claims fax specifically for individual supplemental plans: 877-442-3522. If a claim on that same individual policy needs to be appealed, use 888-659-1023 instead, a separate number from the original submission fax. Both are confirmed on Aflac's own individual claims page. Group certificate claims, the coverage most people actually have through work, follow a different process, typically an employer-specific portal rather than a standing Aflac fax number.
