Voya publishes a disability claims fax, 888-305-0605, confirmed on its own Claims Center domain for both employee and employer/business-owner Disability Income Insurance Claim forms. This route is administered on Voya's behalf by Disability Reinsurance Management Services, but the form and fax instruction live on Voya's own site. Life insurance claims are handled differently: Voya's life claims contact page lists only phone (888-238-4840) and email (Claims@voya.com), no fax number exists for that line.
