What is Oregon WCD's fax number for?

Oregon's Workers' Compensation Division does not take initial injury claims by fax, or by any other direct channel. Form 801, the new injury claim, goes to your employer, who forwards it to the insurer, confirmed on WCD's own worker filing page. What WCD does confirm by fax is the Request for Hearing (Form 2839), stating on the official form itself: "Fax to: WCD Hearings, 503-947-7511."
