How do I fill out the porting LOA? (FAXPLUS_LOA)

A Letter of Authorization (LOA) is signed by the authorized user and/or owner of the number(s). Note that the information on your FAXPLUS_LOA must match the authorized username and service address provided by your current carrier.

Please fill out the LOA according to the guide below:

Name: Your first and last name must be exactly the same as your name on your telephone bill and the name registered in your current carrier.

Business Name: If the service is in your company’s name, make sure to fill this field.

Address: The address must be the same as the address referenced on your telephone bill. The address must be a physical location and cannot be a PO Box.

Current phone numbers and service providers: List all the Telephone Number(s) which you authorize to change from your current phone service provider to the Company or its designated agent.

Once you have filled out the form, make sure to add a signature, print your full name as well as the date.
